The counters at the top show what is Overdue (to do, past its date), what is Waiting to verify (done, not yet judged) and the Effective first time rate for your areas — or the airline — over a rolling 12 months.

The states

Proposed → To do → Done · to verify → Verified effective or Not effective

State	Meaning
Proposed	In a plan that has not been accepted yet.
To do	The plan is accepted; the owner carries it out.
Done · to verify	Marked done, with evidence. Waiting for a verdict.
Verified effective	Judged to have worked.
Not effective	Judged not to have worked. The finding goes back for a new action.
Withdrawn	Taken out of the plan.

Nothing is overwritten

Evidence and verdicts are added beside an action. They never change what they judge, so the record shows every attempt, not just the last.
